Modern Smile In-House Plan
- This is a dental discount plan and is NOT dental insurance
- It CAN NOT be combined with any other dental insurance
- This program is only accepted at Modern Smile Dental. Therefore, if you are referred to a specialist, they WILL NOT offer this discount.
- Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, auto, disability, or workman’s comp type insurance are involved, this discount plan cannot be used.
- If you choose to extend your payment through CareCredit for treatment, the restorative treatment discount is reduced to 5% due to merchant fees.
- You cannot use CareCredit to buy the plan.
- This offer cannot be combined with any other offer.
- Plan fees are only valid when paid at the time of enrollment.
